Requirements
A. Membership to the MEDICAL STAFF Visiting of CAPITOL MEDICAL CENTER is by application or invitation by the Active Medical Staff through the recommendation of the Credentials Committee and with the final approval of the Board of Directors.
Minimum requirements for membership to the VISITING MEDICAL STAFF:
- Graduate of an accredited medical school by the Professional Regulations Commission
- Legally licensed to practice in the Philippines
- Certified specialist and Fellow of specialty societies and in active practice
- Certified of Good Moral Character
B. Papers to be submitted with this application. Kindly bring original documents for authentication purposes
- Letter of intent addressed to: JORGE T. LOGARTA, MD, MMHA, FPSGS, FPCS Medical Director
- Photocopy of Diploma
- Photocopy of PRC Certificate
- Photocopy of updated PRC ID
- Certificate of Residency Training Program
- Certificate of Fellowship Training (if applicable)
- Specialty Board Certificates
- 3 Letters of Recommendation preferable from active CMC staff with same line of practice.
- 2 pcs. 2×2 & 1×1 recent ID picture (colored)
- PDEA S2 license for Anesthesiologist
- Photocopy of PHIC Accreditation ID
- Certificate of Good Standing from Specialty Society
- Photocopy of Philippine Medical Association (PMA) / Philippine Dental Association (PDA) Membership ID
- Photocopy of Professional Tax Receipt (PTR)
- Photocopy of TIN ID
- Photocopy of Life Support Certification [Advance Cardiac Life Support (ACLS) or Basic Life Support (BLS) whichever is applicable]
